Penderita Hipertensi Harus Setop Konsumsi Garam

Jakarta, hainews.id – Penyakit hipertensi identik sebagai penyakit yang menyerang orang berusia lanjut karena risiko hipertensi semakin meningkat seiring bertambahnya usia. Meski begitu, dalam kenyataannya tidak sedikit kasus hipertensi terjadi pada kalangan usia muda.

Berdasarkan data Riset Kesehatan Dasar 2013 yang dipublikasikan Kementerian Kesehatan RI terdapat sebesar 8,7 persen penderita hipertensi usia 15-24 tahun. Sementara Riskesdas 2018 prevalensi hipertensi berdasarkan hasil pengukuran pada penduduk usia 18 tahun sebesar 34,1 persen.

Dari data tersebut angka kejadian tertinggi di Kalimantan Selatan sebesar 44.1 persen, sedangkan terendah di Papua sebesar 22,2 persen. Hipertensi terjadi pada kelompok umur 31-44 tahun sebesar 31,6 persen, umur 45-54 tahun 45,3 persen, umur 55-64 tahun 55,2 persen.

Ali Baswedan, Sp.PD-KEMD, Dokter Spesialis pada Klinik Endokrin di RSA UGM, menyitir pendapat Kemenkes yang menyatakan bahwa seseorang dinyatakan hipertensi kalau dalam waktu dua kali pemeriksaan dalam rentang satu minggu tensi di atas 140 sehingga jika pengukuran sudah 141 maka sudah masuk kategori hipertensi.

“Tapi sekali lagi pengukurannya harus dua kali dalam waktu satu minggu. Dari definisi Kemenkes seperti itu dalam dua kali pemeriksaan dalam seminggu jika tekanan darah 140 ke atas untuk batas atas dan 90 ke atas untuk batas bawah maka yang bersangkutan sudah dinyatakan hipertensi,” ujarnya dikutip dari laman resmi UGM.

Ali Baswedan menuturkan hipertensi adalah suatu penyakit yang dalam keadaan tertentu tidak bergejala. Tidak ada gejala tetapi begitu diukur tensi tiba-tiba tinggi, namun ada juga yang ditandai dengan gejala sakit kepala, merasa tidak nyaman dan lain-lain.

Tetapi sebagian besar hipertensi, tidak bergejala atau silent, dan itu yang berbahaya. Oleh karena itu, pemeriksaan secara periodik bisa tiga bulan sekali sangat penting untuk dilakukan agar setiap individu mampu mendeteksi sejak awal apakah dirinya ada hipertensi atau tidak.

“Terutama bagi orang-orang yang memiliki keturunan hipertensi, misal dari bapak, kakek neneknya, pamannya, dan memiliki kecenderungan seperti itu maka sebaiknya secara periodik periksa,” katanya.

Ali Baswedan menjelaskan secara umum hipertensi dibagi ke dalam 2 kelompok, pertama hipertensi esensial atau hipertensi yang tidak diketahui penyebabnya yang besarannya mencapai 90 persen. Kedua hipertensi sekunder atau hipertensi yang bisa dilacak penyebabnya yang besarannya mencapai 10 persen.

Menurut Ali hipertensi sekunder ini masih memiliki harapan untuk diperbaiki. Semisal karena kelainan ginjal maka begitu ginjal diobati maka hipertensi akibat ini bisa sembuh, demikian pula hipertensi akibat kelainan hormon berlebih maka jika hormon diobati maka hipertensi akibat gangguan inipun bisa normal kembali.

“Artinya yang 10 persen ini merupakan tipe hipertensi yang bisa diperbaiki, sedang yang 90 persen tidak lagi bisa,” ucapnya.

Terkait banyaknya kasus hipertensi dan menyerang usia muda akhir-akhir ini, Ali Baswedan menuturkan situasi dan kondisi saat ini sangat memungkinkan untuk itu.

Artinya masyarakat saat ini dengan mudah mendapatkan sarana periksa untuk mengukur tekanan darah. Orang tidak perlu harus ke dokter atau pusat layanan kesehatan dengan antri tetapi cukup di apotik atau banyak tempat lainnya.

“Artinya kondisi sekarang lebih mudah menjaring penderita hipertensi karena mudah mendapatkan sarana untuk pemeriksaan. Bahkan, alat pengukur pun bisa didapat atau dibeli oleh siapa pun. Asumsi lainnya menurut saya soal tingginya penderita hipertensi karena kita sekarang ini tinggal di dunia garam atau sumber garam,” jelasnya.

Sumber garam ini, disebutnya, sebagai salah pemicu utama hipertensi. Padahal, hal ini berdekatan dengan kehidupan nyata manusia. Ia terbiasa berada di dapur berikut dengan yang lainnya seperti penyedap rasa atau MSG (micin), berbagai bahan lain yang mengandung pengawet kecap, saos, sambal sachet, camilan, makanan ringan dan lain-lain.

Semua itu merupakan sumber garam yang tentu kandungan garamnya sangat berlebihan. Kenapa garam itu berbahaya bagi penderita hipertensi? Karena garam mengandung Natrium dan Natrium ini memiliki sifat-sifat jahat untuk tubuh.

“Dengan mengonsumsi garam secara terus menerus maka natrium akan masuk sel, pada saat masuk sel maka cairan juga akan masuk kedalam semua sehingga bisa overload (kelebihan ) cairan dan kelebihan cairan ini membuat jantung memompa lebih kuat sehingga menaikkan tensi,” paparnya.

Menurutnya, sebanyak 60 persen mereka yang memiliki keturunan darah tinggi sensitif terhadap garam. Sedang 40 persen lainnya tidak sensitif.

Rilis WHO pernah mengimbau untuk penderita hipertensi sebaiknya mengurangi konsumsi garam kurang dari 5 gram. Imbauan ini tentunya sulit untuk diartikan dalam kehidupan sehari-hari.

“Kita tentu tidak akan mungkin menghitung maka untuk imbauan itu sebaiknya semboyan kita kurangi makan gorengan, kurangi makan kecap, pokoknya jauhi sumber-sumber garam. Makan camilan dijauhi, kalau perlu dalam seminggu intinya dikurangi dahulu. Artinya lidah kita dibiasakan untuk yang anyep dahulu,” urainya.

Hal lain yang bisa dilakukan untuk mencegah hipertensi adalah memperbaiki life style atau gaya hidup. Memperbanyak gerak, mengurangi konsumsi garam, alkohol, tembakau dan rutin teratur makan sayur dan buah.

Menurut Ali sayur dan buah sesungguhnya sumber nabati untuk natrium. Untuk itu tidak makan garam dapur sebenarnya tidak masalah karena permasalahan hanya di lidah yang terasa anyep sebab sudah terbiasa asin.

Khusus untuk penderita hipertensi, Ali mewanti-wanti agar pantang garam. Jika perlu stop garam karena garam sangat mengganggu kerja obat.

“Karena jika garam naik naik dalam tubuh, obat tidak bekerja dengan baik dan efektif, dan ada baiknya cek tekanan darah secara rutin,” tandasnya.
